The High-Fidelity Wraparound (HFW) Case Manager (CM) ensures that the principles of the wraparound process are delivered with the highest fidelity to the model. The position requires Monday - Friday 8:00am - 5:00pm hours and is located in the Salt River Trail Region, KY, covering multiple counties. The role requires at least 30% travel within and possibly outside the region. Responsibilities include completing comprehensive strengths-based assessments, working with the High-Fidelity team to develop and implement service plans, facilitating team meetings, conducting home visits, monitoring services, identifying high-risk factors, coordinating care plan activities, consulting with case managers and health programs, and presenting cases at conferences. The role demands experience in building relationships with families, leveraging community resources, facilitating family teams, engaging youth and families in care plans, and problem-solving within diverse teams. Candidates must live in specified counties, have 1+ years of case management experience with children/youth with Severe Emotional Disturbance, knowledge of foster care, juvenile justice, behavioral health services, trauma-informed care, crisis intervention, and be or become a Certified HFW facilitator within 12 months. A Bachelor's Degree in Human Services or related field is required. The position offers a pay range of $21.10 - $36.78 per hour, with eligibility for bonuses and incentives, and includes comprehensive benefits.
